Referring Offices

Hapy Bear Surgery Center is an Ambulatory Surgery Center specializing in treating children, 17 years old and under, that need to be asleep under general anesthesia for their treatment. Please refer your patients to us that require sedation to accomplish their treatment. After we perform the necessary treatment, we will send them back to your office for their periodic and continuing care needs.

Please provide as much information as possible on the referral form. This helps us provide the best care possible by better understanding the patient’s individual situation and why you are referring the patient to us for treatment.

We encourage each child to have an established “Dental Home” for all their routine and periodic oral healthcare needs. We do not provide this service except for special needs children which will not allow for a periodic evaluation unless they are sedated.
